7.1.2. List of Visits related to AT

• Screening Evaluation Visit

• AT Surgery

• Post-Operative Follow Up Visits

• EAT:

  • Phone contact at Months 2 and 5 (1 and 4 months post AT)

• WWSC:

  • Phone contact at Month 10 (1 month post AT)
  • Clinical contact at Month 12 (3 months post AT)

In addition, the WWSC participants will be re-evaluated for surgical candidacy at Month 8 prior to scheduling surgery, with surgery at month 9 as needed.

Pre and post operative clinical visits or telephone calls will be conducted as per routine clinical care at each clinical site apart from the visits listed above.
